This should be a single typed page attachment regarding question 18 of the I-129F Make sure to sign and date it. Notarized.
5.      Original statements 
(from both the US Citizen and foreign fiancé/e) certifying an intent to marry within 90 days of entering the US on a valid K-1.
6.      Proof of having met in past two years
      The following items will not typically show proof of having met in the last two years however will show proof of an ongoing relationship:
o   Copies of phone bills
o   emails (you can edit personal info with a marker)
o   cards inside the original envelope(to document the date they were sent)
o   also include a copy of engagement ring receipt
Copies of all :
o   airline boarding passes
o   itineraries
o   hotel receipts
o   passport stamps (make sure you can read the dates on the stamps)
o   other documentary evidence that you have met within the last two years. You may want to highlight or place post-it notes indicating the dates and locations on the copies (to make the adjudication easier) for the person reviewing your file.
o   Color Photo's of you and your fiance(e) together. Make sure you write your names, date, and location on the back of every photo. Provide two to five photo's. If you only have a single copy of the photo, then make a color copy and send that. Place photo's in a plastic bag or photo sheet and label the sheet. Note that you may not receive originals of photo's back.

1. wait 3-5 business day or 1 week from after receiving noa2 hard copy
2.  call  NVC (603-334-0700),

- ask the the nvc agent if they already have your MNL number.
- if they tell you they don't have it yet, then you can call them back the next day
- once you have your MNL case number:  ask them if when did they released your file for US embassy here in the Philippines. If they tell you its still in the process then you have to wait 2 days again then  call them back.
- "you need to know releasing date of your physical file so you will know and estimate when you will be illegible to schedule an appointment online".
- Make sure you get 
1) MNL case number and 
2) the day it left the NVC. 


3. you can email the NVC too
- click compose message, write your USCIS case number, petitioner's name,and beneficiary's name.
- attached to that your intent to know if they already obtain MNL case number.
- write them (kulitin nyo) 3 consecutive days.   
I-M-P-O-R-T-A-N-T:  FOR THIS THEY WILL REPLY YOUR EMAIL AND WILL SEND YOU A CONFIRMATION THAT THEY HAVE YOUR MNL CASE NUMBER THAT YOU ARE ELLIGIBLE FOR THE NEXT PROCESS. with your NVC elligibility letter you will be able to proceed to medical just by printing the email confirmation because in this email your mnl case is visible.   NVC email address NVCInquiry@state.gov
 Once you have the MNL Case no.:

- At this point, you can track your case through DHL by following the instructions in this
thread. When you read it, you will see why you need the day your case left the NVC.

How to track case through DHL:

1- when you know from AVR that your case was forward to embassy ( mm/dd/yyyy) lets take an example case forward to embassy March 20th 2009

- then under track list click on track reference
- put the reference in this form  EXP   19   MAR   2009A and click track
- now we've seen its completed in both but we searched in 19th yes because they schedule shipping day before so DHL know in advance when they will ship it so day before will be your exact day for NVC shipping your case to your embassy 
- You can see how many cases left NVC. Cases sent to the Philippines is the one that is yours.

 - After your case arrived in Phil. Consulate you can email NOW the Embassy at IVManilaReplies@state.gov, you will get a reply within 3 days. Ask them if they already have your case, this will also make them input your case number in their system, and will inform you how to schedule.


3.      pay the visa fee at BPI.

      - bring passport  and the money ($350 (to be paid in PHP)).‎15,400 (depends on the exchange rate)
      - make sure they give you your receipt because you will need to take this with you to your interview. (do not lose the receipt. this is important during the interview)
4.      then start calling to schedule your interview after 1-2 weeks, once you have done this, 
     - they will send you an email with your interview appointment verification
     - within philippines (02) 902-8930 and  (02) 982-5555  or check.
     - get on the phone or online every afternoon trying to schedule your interview. and wait for their confirmation email. 
     - find out when they received your case

III.             MEDICAL EXAMINATION AT ST. LUKE’S MEDICAL CENTER EXTENSION CLINIC

- you can have your medical before menstruation and 1 week after menstruation. as long as there's no period yet you can go for your medical. Urine exam is for pregnancy test only as what I've knew of.
-wear loose dress is the best, easy to take off  or any comfortable outfit. jeans are common though. for those with long hair, you can bring clamps but clips are available in the xray room. no worries for that.

For the appointment letter to print:
 email the embassy at IVManilaReplies@state.gov to find out if your case has been received.
That way, if your letter does not arrive in time, you will have some documentation with your case number on it.
To ensure a prompt response, include the applicant’s last, first & middle name, MNL case number, and visa class (K1 visa).

Bottled water. Foods are not allowed, but you can slip through hiding your crackers somewhere in your bag.
10.  i'd suggest to go there at least 7:30am coz you`ll still get done at 12noon to 1pm...to be able to breeze through everything before lunch. But they said going in the afternoon, there would be fewer people and the process would be faster.
- St Lukes is walk in and cost is about 9100 pesos (change without prior notice.this depends on the dollar-peso rate


you`ll be having your medical the whole morning and most likely you`ll get done by lunch time and you will just come back on the next day, to know your result, if you pass then you`ll get vaccinated...they`ll just give instructions afterwards...
- or you can go after lunch too. Less people during that time.
- On the second day of the 2-day medical exam, upon reaching SLEC, you have to hand the paper they'll give you on the first day (like a clearance paper) to the guard-on-duty. There will be different stamps available but if he stamps 'for immunization' means that you passed!

EVIDENCE OF A GENUINE ENGAGEMENT 

1. Evolution of Relationship Letter Cover 'events' with date stamps, describe how the relationship has evolved (without getting acerbic or sappy), describe the communication abilities between you two and what languages are used

- bring the album to the interview just in case you need to show more

if you got problems with properly viewing the whole form :What I did was refresh my
browser until the questions show up. If it doesn't, try to restart your pc, empty your

FAQ:  Hello! ! Just wanna ask if I need to paste or staple the 2x2 picture in the DS-156 Form and sign it before coming to the interview?Thanks in advance and have a blessed day to all.
ANS: Just take the pictures with you, and they can attach it to the DS 156 there at the embassy. No need to do it yourself. You have to sign 2 ORIGINAL DS 156 though before your interiew.
For printing: fill the form first before clicking the continue cause if you click continue without completing the form, you can't edit it anymore.
Once you're done and clicked the continue...the barcode will come out on the printed DS 156
All the DS 156 forms should have the same barcode.. They should match both of the DS 156 forms. DS Forms will be given on the entrance of the embassy together with the appointment letter.
Make sure you pasted your ID pictures with the 2 copies of the DS forms and place your signature and write down the date of your interview. This is only for DS 156 not DS 156K. Do not affix your signature in DS 156K. You'll sign it in front of the Consul.
Your ID pictures will not fit the box on the DS 156. That's normal. Aside from these ID's pasted here in DS 156 and on the medical, no other ID's are needed. Make sure it's 2x2 US visa Id picture. The face on the US visa ID pictures is bigger that our normal 2x2 ID pictures.

DS-156 must be fully completed and printed from the website http://evisaforms.state.gov. When you click "Continue," the site generates a unique barcode containing all the info on the form. If you leave blanks to be filled in, the barcode will not correspond to the form data. The consulate may well reject this form and send your fiancee out to redo and reprint the form, leading to delays.

DS-157 is not an online form. It can be fully completed in pen or ink. DS 157 can be completed online too and have it printed.
Yes, DS-157 can be downloaded in PDF format and completed on the computer. Filling it out on the computer is not required, however. It can be filled out by hand in part or in full.

ANS: In my case, my fiance had more than the poverty level too but we were told to include bank accounts and his real properties, because these items also appear in the Income tax return. my visa was approved without too much questions from the consul.

Track you visa at 2go website using your passport number or a special number given to you by 2go. email the US embassy stating your passport no., receipt no., interview date and ask them of the status of your visa 5 days after your interview. Here's the link to the 2go website:

For opening the visa packet: just cut a small slit on the outside plastic enough to get your passport. Keep the package intact for the POE. Immigration officer in the POE will be the one to open the yellow packet. This is very important.

ANS: Just go to USEM nlng Sis and don't even try to staple it yourself as it may seem to be tampered na and that will 'cause another problem. It shouldn't be a big problem nman siguro, I got my visa today too and di ko na tlga tinanggal from the pouch yung yellow folder,kinuha ko lang yung passport ko by making a little slit sa gilid. 2 staples lang kasi yung ginamit it will be easily detached tlga. charge a lot (P5/page) for the photo copies so I suggest you bring 3 photo copies of your visa & passport before going there, plus the original.
Prepare to answer a number of forms: 1 for the counseling (back-to-back), 1 for CFO (back-to-back), 1 for Metrobank card, 2 take-home forms and 1 survey form. Bring your fiance's info sheet. You will need data like his mother's maiden name, zipcode, his college or vocational degree, his parent's contact info if he doesn't have a landline. They will require you to give 4 contact numbers (2 from you and 2 from your fiance) - if you don't have a landline you can include your parent's number or your closest friend. You can bring your cellphone to copy contact info or text/call your fiance if you have questions.
Eat before you go there, you will be hungry once it's over.

- People behind the window will call name one by one according to the list on that paper that seminarians log in.
- make all documents ready and arranged it accordingly:

- Divorced Paper of my husband (Original and Photocopy),

- Photocopy of two identification (ID with your picture - I photocopied my PRC ID and POSTAL ID,
- Fiance/Husband passport and ID (Photocopy).
Sometimes they will ask if you are the first person that my fiance/husband petitioned and I said YES,
and she handed me the form to be fill up and I paid P250.00 pesos and go back to my chair and
fill up the form


Note: Some documents are also very important as what I observed:


Copy of the legal capacity
- Copy of the passport of your fiance/husband
- Copy of Immigration stamped at your husband's passport if he married a Filipina more than once; in some case they just want a copy of it to check your husband's arrival in the Philippines.(The nun will explain it during the seminar).
- Copy of your visa (If you already have the visa)
- Copy of husband's or fiancee Birth certificate
- Copy of Husband's or Fiancee Passport


fill up the form and make sure you do know your husband/fiance's important details or memorize 
your husband's /fiance's whereabouts. (Note: You need to know the information about your
husband or fiance well. His name, birthday, address,work, work address, educational attainment,
course, how many times he got married (if divorced), his arrival in the Philippines,how you met,
where you met, etc) You can finish filling up the form quickly and save your money if you did this.
You can observe during the seminar that some of the fiancee and wives don't know what is the
work of their husband/fiance and work address. They Mostly relied to the papers that they are
holding (like copy of passport, birth cert, marriage cert) where in some personal info their
husband's or fiance can't be found.Some made a call to their sweetheart abroad to ask about
these things and it cost money of course.(Phone calls)


- While the rest still filling the form, the guy behind the window call names again. This time it's for
picture taking.
- After Photo taking, everyone will  gather in a room with a TV. Firstly, the nun showed us a
clippings that they gathered from "Magandang Gabi Bayan" as ANdz and FAith mentioned. It
isn't that bad though. It will makes us awaken to the real world. It is a sort of reminder, warning,
since we are going to the foreign country. Mostly,  Complains and report of Filipina abused by
their foreign husband. But nothing to be worry or afraid of if you only know the person you will
marry or get married in most part. Yes, the nun was little bit strict. She is trying to put or planted in
your head and heart that we married a foreigner because of "LOVE" and not for money. That our
main purpose is to be a Wife. I didn't regret that I attended the seminar, I learned a lot like
upgrading ourselves, love our husband, etc.....The seminar will last for more or less 2 hours. After
that, all american fiancee and spouses stayed in the room and the fiancee and spouses of other
national went to other room. for USC fiancee and spouses, you will stay.  Another woman will
give a briefing about American culture, port of entry, different kind of visas, visa adjustment,
etc.....


- Lunch break


- After Lunch, some will be called back to go inside to finish filling their forms. While others will
be called by a nun and another woman for individual counseling .Be sure to fill out the forms early
so you will not be those who will be called again.


- Some will be interviewed by  a Nun others like social worker. It wold be fine. She will ask
questions about your husband/fiance, his arrival, how you met, where you met, how long have you
met, if ever he was been married before, how many times he went to the Philippines, etc.....Very
simple questions if you only know your husband or fiancee well. Big age difference is not really an
issue.  Nothing to be afraid. Just know your honey well. :D


- The Nun will approve and you pick up my SMEF-COW Certificate (BLUE) from the guy
behind the window and he will advise you to go to CFO Office to pick up the CFO
CERTIFICATE (Green).

- In going to the  CFO Office you can ride a jeepney. The woman told us to photocopy our
SMEF-COW Certificate (blue) and went to her with our P400 pesos and she issued us the
CFO certificate (Green).


- If you have your passport with your visa they will put the sticker to your visa automatically. But if
you don't have the visa yet, they will only give you the Green CFO CErtificate. CFO Sticker can
be claim either in Cebu or Manila if you get your visa. (Don't ever lost your CFO Certificate
(Green) as you needed it to get your CFO Sticker stamped or attached to your visa before
departure to foreign country.

then download the necessary form 
that she needs to fill out and she also needs to fill out one form too when she get there
- after that, she should pay the fees :) no need to bring pictures if you go to that place.
- Cebu only accepts 15 for a seminar, but some said they could accommodate more than 20
there in all, although there were only 8 of them in the American seminar (for those traveling to
U.S.)
hello... for those who attend the cfo seminar just want to ask if i should bring the Immigrant Data Summary for USA-bound emigrant?.. but im confused because it has instruction that it must not be detached.. should i photocopy it?.
photocopy your Immigrant data summary...do not detach it from the yellow packet as you will have difficulty at the POE immigration...the security guard at the CFO told me to be careful with the immigrant data summary ...goodluck!
For POE:
At the airport, you need to have handy access to the passport-with-visa, and the do-not-open packet (which is surrendered at Immigration). You'll have been told to be ready to present the chest x-ray, but they probably won't care. That's all you need for an airport PoE.

The most important thing at an airport PoE is to ensure, before leaving the customs area, that you have an I-94 form stapled into your passport, and that ALL of the information on it is 100% correct. Triple check the spelling of your name, and anything else written on it. Correcting it later is a pain in the rear.

Definition of terms:

AOS - Adjustment of Status - Applied for after entering the US on a K1 visa and marrying within 90 days. Changes status to "conditional permanent resident" and you will be sent a green card. This process takes around 90 days however it can take MUCH longer.
AP - Advanced Parole - Allows you to re-enter the USA while your AOS is pending. You are allowed to leave whenever you want, but doing so without the AP document (or green card) means you will not be able to re-enter. Leaving without it also results in your case being considered "abandoned".
CO - Consular Officer - In this thread related to the person actually processing my documents
EAD - Employment Authorisation Document - Allows you to work whilst the AOS is being processed, as long as you have a SSN of course. Once you received your GC your EAD is no longer needed
GC - Green card - short-form for green card so I don't have to keep typing it :)
IO - Interviewing Officer - The person who will interview me (if needed). Apparently not every case requires an interview. Lets see if I can be one of the few who don't need one :D

RFE - Request for further information - Received if something you mailed in is incomplete or missing. It doesn't cancel your case, just slows things down a fair bit, so doing everything to prevent getting one is your aim:D

The guides suggest doing a cover letter. Personally I'm doing a cover letter for each separate case (AOS, EAD and AP). I have read that the CO's don't tend to read the cover letters so really why bother, BUT I'm not doing it for them so much as for me. Filing so many documents means a lot of stuff to remember and I also think it looks neater and prettier. Also, while reading through the instructions I go through and fill in the cover letter with the attachments needed so I don't forget anything.

Also, remember to check the filing location as it may have changed since the form was made. In this case it tells you it depends on the category you are applying under, on other forms it might say "check the form" or "check the instructions".

The two items listed below are on the VJ AOS Guide, however NOWHERE on the forms, or their instructions is it mentioned that these items are required. These items only serve as additional information. You are required to bring your passport to the interview (if you have an interview) so perhaps by mailing this information you might not need to have one. Ultimately more info is better than less info (within reason of course) so it can't hurt. Personally I'll be including them in my parcel (in my attempt to avoid an interview :P) but it makes sense that they're not needed as your Alien number is your identifier now (where with the K1 your passport number and then case number were).

Note 2: There is a huge point of contention on VJ regarding whether or not to send DS-3025 or if you will require form I-693 (immunisations only). The reason for this is, the paperwork clearly states that if you COMPLETED your immunisations PRIOR to your K1 interview in your home country (and thus your medical information is complete in the paperwork you handed in at POE (and subsequently forwarded on)) then only the copy of the DS-3025 is required. HOWEVER some people who assumed their info was complete and sent in the DS-3025 received an RFE requesting they submit a form I-693. Further information is mentioned on this VJ thread: http://www.visajourn...howtopic=101202 So whether you go to the trouble of obtaining one so you don't risk and RFE and thus a slow-down is up to you. Personally I am in the process of calling Civil Surgeons (CS's) in my area to find our their fee. If the fee is reasonable we might do it just so we don't risk the RFE but at the same time i honestly think my form is fine and I have all immunisations filled in, either with dates or "not age appropriate". I'll let you know what I decide and then what the outcome is. Remember though every case is different, no matter how similar they might be.

Note 3: Tax information is only REQUIRED for the most recent tax year. Question 25 on the I-864 still requires that you indicate your earnings for the most recent tax years, whether you provide evidence of this income or not. There are two tick boxes in that question, where you can indicate whether you are only including the most recent tax year, or the three most recent. I personally prefer to send in IRS transcripts so that you are sure that you have not excluded any tax information and I also prefer to send in all three years of tax information just to "be safe". As we're applying together with the AOS then the following isn't REQUIRED, however if you choose to include it, it's up to you, like i said before, more info is better than less info

Note 1: Being adjustment applicant's, we're under category c9. You will see there's also a category for K1 and K3 visas (a6) but this is for when you're not applying at the same time as the AOS.

Note 2: It actually says "If no prior EAD has been issued, you must submit a copy of a Federal Government-issued identity document such as a passport..." as well as some other examples but personally I think the passport is the easiest one.

Note 3: The next issue for me (being female, married and changing my name) is my passport is still in my maiden name. For that reason it seems to make sense to include a copy of my marriage licence. Although these things are included with the AOS application, they can always shred multiple copies :D

General

An important thing to remember is if you change your address at ANY stage while you are an LPR (or their sponsor) you are required to change your address within 30 days of moving. There are penalties for not doing so. The USCIS has an online change of address service but you STILL need to actually post a form to them. Here's the site: https://egov.uscis.g...i/go?action=coa . The reason for the paper form is so it's changed on the masterbase.

1. POE
Don’t forget to bring the brown envelope from the embassy and hand it to the immigration officer, together with your i-94. Make sure all details on the i-94 are written legibly and correctly. Also, do not leave the airport without checking if all the details on your i-94 is correct (Your name, alien number, date of stamp etc) and DO NOT LOSE YOUR I-94.

2. GET A MARRIAGE LICENSE
Check your state laws for the requirements to get married.. Go to your local town clerk to get the license. Some may need a SSN, some won't. So you can interchange #2 and #3.

3. GET A SSN
K1 visa holders are allowed to get a SSN as long as they still have a valid i-94. It is recommended to go 10-14 days upon entry so that by then you should already be registered in the database. Now I have been told that there are some SSA who are clueless about k1 visa holders and refuse to give them the SSN until you have your green card. This is FALSE, and if this happens, ask for higher management and demand for it, because you are entitled to one.

Things to bring: Passport, I-94, NSO Birth Cert, SSN Form (you can get one there). If you go when you are already married, bring your MC.

It should arrive in 1-2 weeks.

4. FIND A CIVIL SURGEON

Only if your DS-3025 is marked “incomplete” is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon’s office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon’s Certification). They will then seal the form in an envelope and return it to you.

So this part is a little confusing because I have all the complete age-appropriate vaccines, and yet some people claim to have gotten RFEs for not having their I-693 transcribed so we decided that were gonna do it anyway to avoid any complications The trick here is to call around and find a civil surgeon who will not require you to do another medical, because your medical exam should still be valid for a whole year!

5. GET MARRIED AND GET THE (certified) MARRIAGE CERTIFICATE

The marriage certificate is important for AOS, so make sure you grab copies of it!

In the checklist above it is recommended that you include a Cover Page with your application. While this is optional, filing a cover page will help the USCIS understand what benefit you are applying for and specifically what items you have included in your package. Again, make sure the cover page includes:

PAYMENT
Personal checks must be pre-printed with the name of the bank and the account holder. Also, the account holder’s address and phone number must be pre-printed, typed or written in ink on the check. All checks must be typed or written in ink.

1. Write the date you are filling out the check including: day, month and year.

2. On the “Pay to the Order of” line write: “U.S. Department of Homeland Security.”

3. Write in numbers the exact dollar amount of the fee for the service you are requesting. In the example, the amount is “$595.00.”

4. Spell out the exact dollar amount of the fee for the service you are requesting. The “cents” portion of the amount should be written as a fraction over 100. In this example, the amount is “Five hundred ninety-five and 00/100.”

5. Write a brief description of the purpose of your payment. In this example, it is “N-400 application fee.”

6. Sign the check using your legal signature.

IMPORTANT!
Make TWO copies of the entire package before you send it in. This includes the money orders too. You want to have a perfect replica of the package you are sending in. All Forms that you submit must be originals with original signatures. Supporting Evidence that you submit may be photocopies. Retain ALL original supporting Evidence since the USCIS has the right to check them by issuing an RFE (Request For Evidence). If you receive an RFE, follow the direction exactly, and make two copies of what you sent back. During any future interviews the USCIS may also want to examine the original supporting evidence.


Once you send it in, here is what to expect.

You will receive a NOA1 for all 3 applications, followed by a biometrics appointment. After that you either get an interview date or you get transferred to CSC and you'll be approved with no interview. But don't worry, I heard that the interview is not hard at all, so don't feel bad if you get one!

tawag ka lang sa company and tell them you want the bills in your name, all you need to do is provide your SSN and some ID, probably the Mil ID will be okay. Since wala tayo credit history, you might be required to pay a deposit ( I was required) but if you pay on time consecutively for 12 months then it will be returned to you with interest, so not bad. :) Buti ka pa may credit card ka na, ako naman ayaw ni hubby na we have separate credit cards so he'll just make me a joint owner dun sa kanya, and in a few months pag na-build na credit score ko, dun palang ako mag-open ng sarili ko.
